Credentialing Application
The Request for Credentialing Application is the first step for a provider to request privileges at U.S. Renal Care. Once submitted, we will email the provider a link to the credentialing application and establish their CredentialStream account.
CredentialStream Hub
Once the provider has an account established, CredentialStream Hub may be used by the provider or the provider’s administrator to access their medical staff file to submit or update information.
